Staff Medical Forms

The medical report must be completed by a physician and sent to the office before June 1. Staff from Canada must bring their Government insurance card.

Medical Insurance: It is very important for every staff member to come to camp with insurance coverage. Please make sure that if you are not covered with your medical insurance in Canada to buy a travel insurance plan. This is not expensive but vital. If Chas Veshalom you are in a medical emergency without insurance it can cost you a lot of money. Camp Pardas Chanah is not responsible for any medical emergencies.
